Initial Animal Chiropractic Visit

Subjective/History: Ollie was seen today for issues reported by their owner/handler. Ollie has been stiff when trying to move around and demonstrating other complications. No recent changes in bowel or bladder habits. Appetite is normal. The owner/handler reports that Ollie has been experiencing these issues for an unknown duration and it began gradually over time. Ollie has no known history of surgery. Ollie's not currently taking medication and/or supplements. The owner/handler denies any knowledge or history of X-rays for Ollie. Ollie has not had chiropractic care before. Additionally, Ollie has received yoga .

Objective: The patient presents with an alert disposition status. The patient's head position is observed to be normal. The patient's top line is observed to be normal. Ollie's seated posture is observed to be with both legs out.

Motion palpation of the spinal segments and extremities revealed the following chiropractic listings of segmental hypo-mobility (vertebral joint restrictions): Ribs:123, 234, 345 • L ELB jt:Extension

Assessment/Goals: Chiropractic adjustments to the adjusted areas were well tolerated. I expect continued positive outcomes with additional care. Patient goals include the following: no goals selected.

Plan: The discovered vertebral subluxation complexes and associated intersegmental, biomechanical, hypomobile chiropractic listings were reduced.

Ollie is currently in the acute phase of care. I have recommended the owner/handler continues to ensure Ollie is seen at the previously recommended treatment frequency.

In the best interest of optimizing Ollie's health, the following home care instructions were recommended: They should be fine.
